Social Worker /Case Manager
HOPE Outreach Center Inc., a 501(c) (3) non-profit community outreach agency serving families in the South Broward area,(FL) is seeking a self-directed individual to serve as Social Worker/Case Manager. The individual shall consider a holistic approach when serving clients. This includes but is not limited to performing a bio-psychosocial intake and assessment, developing case plans, making referrals and links to appropriate community services, and monitoring client progress to ensure that services are having an impact on the problem. The Social Worker/Case Manager will also be responsible to provide educational opportunities to meet client, community, and organizational needs.
Requirements:
The ideal candidate will have Bachelors or Masters Degree in social work, social services, or related fields and a minimum of 3 years of related experience and/or training. The person must be self directed and skilled in handling multiple tasks and caseload, be energetic, caring, and creative, with above average written and verbal communication skills. Bi-lingual (English/Spanish)required. Benefits do not include medical/dental.
